CrossOver Healthcare Ministry is a 501(c)(3) non-profit organization that operates two healthcare clinics providing low-income, uninsured and medically underserved residents of the Metro Richmond area with high quality, compassionate healthcare services. Since our founding over 35 years ago, CrossOver has grown immensely in order to address the increasing need for accessible and affordable healthcare services. CrossOver provides a medical home to our patients, offering an innovative integration of health services under one roof, including primary and specialty care, chronic disease management, medication and medications management, dental, vision, pediatrics, mental health counseling, OB/GYN, HIV diagnosis and treatment, social work case management, and health education.
Full HIV/AIDS care
CrossOver is the only charitable clinic in Richmond to offer full-spectrum HIV/AIDS care, including testing, diagnosis, care, prevention and education. Rapid confidential HIV testing is available on a walk-in basis at both of our clinic locations. Testing is available to patients as well as non-patients Monday through Friday, 9 a.m. to 4 p.m., and after hours testing is available on certain dates. Primary and Specialty Care
CrossOver is a medical home to our patients, providing a wide range of healthcare and wraparound services under one roof. Patients receive primary care to address and identity any health concerns. CrossOver patients can also access specialty care, including orthopedic care, neurology and cardiology, either through our on-site volunteer providers or through our partnership with AccessNow. All patients receive an annual depression screening and, if appropriate, referral to our mental health program. Patients also have access to the medications they may need through our on-site pharmacy.
CrossOver offers a weekly hospital discharge clinic to link new patients with resources necessary to manage their health after an unplanned hospital stay. The team-based, comprehensive care model includes an intake appointment, social work visit for case management, health education with a nurse, primary care provider visit to discuss care, medications and referrals, and fill prescriptions. The accelerated patient intake promptly meets the complex needs of patients and provides access to ongoing, preventative primary care to prevent hospital re-admissions.
CrossOver identifies the most vulnerable families in our patient population to receive intensive case management services. Our staff social workers and dedicated interns work to connect these patients with community resources such as food banks, legal assistance, employment opportunities, domestic violence services, and housing programs. All new patients are given a social work assessment upon intake to immediately address any needs.
CrossOver provides assessment, diagnosis and treatment services for patients with mental health concerns. The treatment services available include ongoing psychotherapy and medications management services.
CrossOver’s pediatrics program utilizes our integrated care model to quickly provide children with the healthcare and wraparound services they need to thrive. CrossOver is the only free and charitable clinic in central Virginia to provide a dedicated pediatrics program to uninsured children. Pediatric patients receive age-appropriate vaccines at primary care appointments along with culturally competent health education designed for the entire family. Children are also fast-tracked to receive on-site preventative and emergency vision and dental care, and receive intensive, trauma-informed case management services through our social work program.
CrossOver patients can receive a complete eye exam, including retinal screenings, refractions, and visual field exams, through our vision program. This program was developed in collaboration with Virginia Eye Institute and the Medarva Foundation Fund. Our vision services are available to CrossOver patients, as well as to patients from other local clinics, including: Goochland Cares, the Center for Healthy Hearts, St. Joseph’s Outreach Clinic, the Free Clinic of Powhatan, the Richmond Behavioral Health Authority, and the Capital Area Health Network. Through partnerships with Lens Crafters and Virginia Eye Institute, CrossOver is able to provide glasses vouchers for patients who need corrective lenses.
Our dental clinic provides dental hygiene cleanings, screenings, fillings, extractions, and dentures to CrossOver patients. The dental program places an emphasis on oral health education and collaborates with our primary care providers to help patients understand the connection between oral and overall health. The dental program has also initiated a new project, the Dental Referral Network, which allows staff to refer patients to external dental offices with dentists who are willing to provide pro bono care to our patients.
CrossOver operates an on-site licensed pharmacy so patients have the ability to access the medications they need. The Community Pharmacy is the only licensed pharmacy operated by a charitable clinic in the Richmond area. In FY 2020, the pharmacy provided 30,213 donated medications worth a total of $9,304,374 to patients of CrossOver and of other area free clinics including Health Brigade, the Richmond Center for High Blood Pressure, Goochland Cares, the Free Clinic of Powhatan, Bon Secours Care-A-Van, and St. Joseph’s Outreach Clinic.
Chronic Disease Management
CrossOver provides comprehensive treatment plans for patients suffering from diabetes, hypertension, chronic obstructive pulmonary disease, cardiovascular disease, and other chronic conditions. Through a partnership with the VCU School of Pharmacy, we also offer intensive diabetic management through our ambulatory care clinical pharmacist (PharmD) program.
