Honoring Choices Virginia was founded by local health systems and the Richmond Academy of Medicine. We work to encourage advance care planning conversations among individuals, families and healthcare providers throughout our communities so that individuals’ healthcare choices are best honored. Our partners provide quality advance care planning conversations for community members and educational programming, at no charge.
