DPHS Healthcare Centers
DPHS operates two health centers. We serve individuals regardless of one's housing or insurance status and services are based upon one's ability to pay. Our two health centers provide care in four clinical areas: primary health; behavioral health; dental; and vision. In 2017, health services were provided to 7,227 men, women and children for nearly 38,000 patient visits.
Safe Haven is a 21-bed free-standing facility that offers transitional housing, comprehensive and integrated health and support services to individuals with mental illness and/or substance use disorders. Safe Haven provides 24-hour staff supervision and guidance with the ultimate goal of transitioning clients into permanent housing and independent living.
Medical Respite is a 20-bed program designed to provide convalescent care and emergency shelter upon hospital discharge for up to 30 days for medically frail adults who do not have appropriate living arrangements to support recuperation.The program provides 24-hour staff supervision with the ultimate goal, of establishing a medical home and attaining stable housing. Co-located with Southside Health Center.
