THIS POSITION IS PART TIME

SUMMARY: This position provides administrative assistance and training to staff and case management services to clients as needed for programs under the Aging and Disability Services Department.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Provides case management services for individuals under Developmental Disabilities Waiver (DD Waiver) Guardianship and Care Coordination programs, including assessments, home visits, case documentation, referrals and services as needed.
Prepares appropriate documentation for case files, billings, logs, and personnel training records complying with DD Waiver and Guardianship local and state regulations and internal and external entities’ policies and procedures.
Prepares letters, reports, checklists and other correspondence in a timely manner and as directed.
Maintains and organizes purged, existing and closed case records for Aging and Disability programs.
Attends and participates in all other appropriate agency and program meetings and trainings related to performing job functions as required.
Provides back up for Care Coordination, Guardianship and DD Waiver in staff absence.
Develops and conducts new staff orientation and training for department staff as needed.
Maintains DD Waiver waitlist and on-board new DD Waiver clients.
Assists Guardianship Supervisor with conservator and representative payee cases to ensure client accounts are properly managed.
Represents the Agency in the community and workplace in a professional and ethical manner.
Demonstrates sensitivity to the service population’s cultural and socioeconomic characteristics.
Demonstrates strong interpersonal, decision-making, problem-solving, oral, and written skills.
Utilizes a basic knowledge of Microsoft Office and Outlook applications.

Required Qualifications:
EDUCATION and/or EXPERIENCE: Bachelor’s Degree in the human services field is required and possess developmental disability work experience (OR) the knowledge, skills, and abilities set forth in VA code 12VAC30-50-440.
