CrossOver Healthcare Ministry provides compassionate, high quality healthcare to the uninsured and medically underserved in our local community through the operation of two healthcare clinics; one in the City of Richmond and one in Henrico County. CrossOver is a 501(c)(3) non-profit organization that offers comprehensive healthcare services to its patients, including: primary care, specialty care, pediatrics, dental, vision, behavioral health, HIV/AIDS care, women’s health & OB/GYN, medications and medications management, and social work case management.
Position Summary:

The Bilingual Community Health Worker – OB Program (CHW) will work directly with low-income, pregnant women to ensure they receive care. The CHW will assist with patient navigation, care coordination, health education and linkage to other community resources. The CHW will also provide education and follow up regarding COVID-19 vaccines and health and safety information for patients.

Key Responsibilities of Position:

• Help patients and their family connect to medical and social services by phone, email and in person
• Help patients and family members develop health management plans and goals
• Screen individuals for health-related social needs and primary care access
• Follow-up with patients to confirm linkage to services and completed referrals
• Follow-up with health management and care plans with both patients and medical providers
• Help patients and families access resources: scheduling appointments, completing assistance program applications (i.e. WIC, SNAP, Medicaid, etc.), linkage to other related health-related services as needed
• Motivate patients to be active and engaged participants in their health and wellness
• Help educate patients on the importance of healthy lifestyle choices overall and as related to COVID
• Provide basic health education information overall and as related to COVID
• Assist with developing effective marketing and outreach materials for patients
• Provide interpretation services for program patients at healthcare appointments at CrossOver
• Effectively work with people (staff, patients, patient’s family, volunteers, doctors, agencies, etc.) from diverse backgrounds as a team
• Use electronic health records systems to update and track patient information and referrals, including eClinicalWorks and Unite Virginia
• Linkage to community resources
• Service system access and navigation
• Care coordination and management
• Build community relationships with schools, churches, social service agencies and other organizations

Hours: 40 hours per week. Generally, 8:00 a.m. – 4:30 p.m., but some evening and weekend work will be required. This position will be located at CrossOver’s Richmond Clinic at 108 Cowardin Ave.

Required Qualifications:
Required Qualifications: • Bilingual (Spanish) • Associates or bachelor’s degree in a health and human service field • Experience with working directly with diverse population • Excellent oral and written communication skills • Computer skills and use of handheld tablets • Experience with working with low-income, disadvantaged or vulnerable populations • Excellent interpersonal skills required and ability to interact professionally and gain trust with culturally diverse individuals • Prior experience with community resources and social services in the greater Richmond area • Ability to show empathy • Ability to handle confidential information with discretion and professionalism • Basic CHW training will be provided to all hires to fulfill their roles. Successful participation in CHW training is a condition of continued employment Preferred Qualifications: • Prior experience and training in a CHW or similar role preferred but not required • 2 years of work experience in a health or human services preferred but not required
$37,440 – $47,840/annual ($18-$23/hour) depending upon experience
