Partnership for Families

The Partnership for Families Social Worker’s role is a professional position coordinating case management in collaboration with families and partners. The Social Worker is also responsible for working collaboratively with families, providing family assessments, creating and monitoring plans, making referrals. Conduct and providing support to community engagement activities including community outreach. The PFF Social Worker is also responsible for supporting program staff in providing programmatic offerings as directed by PFF, community need and organizational strategy.

The PFF Social Worker participates in the data collection, review and monitoring of family data to ensure compliance of programs to established guidelines and provides appropriate analysis and recommendations to PFF for enhancements or change. The PFF Social Worker will often represent the Partnership at community events, partner meetings and work groups as needed and directed.

LEVEL OF RESPONSIBILITY

-Facilitates collaborative case management with PFF partners, specifically case management staff, and community agencies to support the needs of families and children
-Assesses families and their needs to ensure the right services are available and utilized
-Assist with data collection, data entry and coordination.
-Works collaboratively with family to identify goals and create family plans, monitors and supports family progress against goals.
-Provides holistic, longer-term support to families to ensure their transition to a better, stronger quality of life is maintained
-Acts as first point of contact for all incoming phone/internet and in person inquiries for services and referrals.
-Maintains contact with service providers regarding family progress
-Coordinates and supports outreach efforts.
-Supports families and identify and refer to outside agencies in the following areas social service needs, including but not limited to, developmental delays, housing, health insurance, public assistance, hunger, and other community resources and services; assists families with transition to school activities including providing early educational resources; prepares and maintains records and reports related to children and families; participates in and encourage parent involvement in all NCFL programs, activities supporting community awareness, parent trainings, and events.

REPORTING STRUCTURE:

The PFF Social Worker is a direct report to the Program Director .

MINIMUM QUALIFICATIONS

Education: Minimum of Bachelor’s Degree, preferably with social work major or concentration; Masters Degree Preferred; Strong knowledge, training and/or experiences with early childhood education, adult literacy, family literacy, mental health and social services preferred

Experience: Minimum of 5 years’ experience in social work/social service provision preferably in the non-profit arena
Technical Skills: Must have a strong knowledge of Microsoft Office Suite as well as other appropriate technical software packages. Specifically, staff member must be proficient in Microsoft Word, Excel, PowerPoint as well as other productivity applications.

Flexibility: Must be able to work a flexible work schedule as dictated by the programmatic needs of the Partnership. Ability to work evenings and/or weekends as appropriate.

Utilizes various treatment modalities of individual and group work, casework, community organization, and community outreach activities to alleviate or remove problems that interfere with family strengthening

Provides a full range of social work services within commonly accepted standards of social work practice, which includes case work, individual, family and group counseling, case management and other services as appropriate.
